Position: Four (4) Job-Hire Driver (Ambulance Driver) 

Interested and qualified applicants shall submit or send the following documentary requirements to DR. JUDY ANN T. DUMAYAS, Medical Center Chief I, Department of Health – Western Visayas Sanitarium, Brgy. Inangayan, Santa Barbara, Iloilo:

1. Application letter addressed to the Medical Center Chief of Western Visayas Sanitarium;

2. Four (4) copies of duly accomplished Personal Data Sheet ( indicating education, work experience, number of hours of trainings/ seminars attended, eligibility);

3. Authenticated copy of Diploma, Transcript of Records (for new entrants/applicants);

4. Copy of Certificate of Trainings/ Seminars attended;

5. Authenticated copy of a.)Career Service (Professional) Second Level Eligibility/Board Rating/RA 1080 for Second level positions b.) Career Service (Sub professional) First Level Eligibility for First level positions c.) Copy of Professional Driver’s License for Driver II positions ;

6. Two (2) copies of Performance Rating for the last two rating period ( for applicants applying for promotion)
